About Always Culture

Since January 2017, Always Culture has served as a strategic partner for healthcare organizations striving to bridge the critical gap between clear clinical intent and the patient’s actual perception of care.

Headquartered in Montgomery, Texas—neighboring the diverse medical innovation of the Greater Houston area—we were established to challenge the status quo of traditional patient experience training.

We recognized early on that while frontline staff work tirelessly to provide safe, high-quality care, these efforts often fail to translate into the specific metrics measured by regulatory surveys.

Our organization was built to solve this disconnect across the entire continuum of care. While our foundation is rooted in improving HCAHPS scores for acute care hospitals, our behavior-based methodology is equally effective for Ambulatory Surgery Centers (ASCs) and outpatient clinical settings.

Whether navigating the complexities of Value-Based Purchasing (VBP) or preparing for the specific nuances of OAS-CAHPS and CG-CAHPS, Always Culture equips leadership and staff with the precise tools—not just to improve scores, but to foster an environment where patients feel genuinely heard and respected.

Mission, Vision, & Values

At Always Culture, we believe that exceptional patient experience is not achieved through slogans or abstract theory, but through the consistent application of specific, tangible behaviors. Our organizational identity is defined by a commitment to the men and women at the bedside and the patients they serve.

Our Mission

To equip healthcare organizations—from acute care hospitals to ambulatory surgery centers and clinics—with practical, behavior-based training that empowers staff, improves patient outcomes, and measurably elevates CAHPS performance.

Our Vision

We envision a healthcare ecosystem where the gap between clinical intent and patient perception is closed—where every patient feels safe, heard, and respected, and every caregiver feels confident that their hard work is accurately reflected in their organization’s scores.

Core Values

Respect for the Frontline

We fundamentally reject training that relies on guilt or shame. We believe healthcare professionals wake up every day aiming to do their best. Our role is to empower them with the tools they need to succeed, not to criticize their current efforts. We honor the difficulty of their work and design solutions that support them.

Evidence-Based & Behavior-Driven

Compassion is the heart of care, but communication is the vehicle. Our methodologies are not formatted as vague “soft skills” but as concrete, reproducible behaviors. We ground our training in peer-reviewed research and align it directly with regulatory surveys—including HCAHPS, OAS-CAHPS, and CG-CAHPS—to ensure that improved culture leads to improved data.

High-Fidelity Communication

In an era of digital fatigue, quality matters. We value high-fidelity storytelling and broadcast-quality production because we respect the learner’s time. By utilizing engaging media rather than dry lectures, we ensure that critical concepts are retained and applied, bridging the divide between learning a concept and living it on the floor.

The "Why": Our Origin Story

The genesis of Always Culture lies in a unique intersection of clinical reality and media strategy. Our Co-Founder, Matt Dufilho, spent years navigating two distinct worlds: one as a Physical Therapist delivering acute care in trauma centers, and the other as a Director of Media Production for a major health system. It was at this crossroads that he identified a pervasive and frustrating disconnect in healthcare. He saw frontline staff who were deeply compassionate, highly trained, and working tirelessly, yet their facilities were struggling with stagnant patient satisfaction scores.

The “aha moment” arrived when Matt realized that the issue wasn’t a lack of care, but a lack of translation. Patient survey instruments—like HCAHPS and OAS-CAHPS—rely on specific, nuanced wording to measure experience. A care team could provide medically excellent treatment, but if they didn’t communicate in a way that aligned with how patients were being surveyed, the data wouldn’t reflect the quality of their work.

Matt knew the solution wasn’t more clinical drills; it was to teach the “unwritten rules” of patient perception. He recognized that by equipping staff with specific, behavior-based communication tactics, organizations could bridge the gap between their intent and their results. Joining forces with Matt Brannon, an operations leader from Cedars-Sinai with deep experience in high-volume patient flow, and Casey O’Quinn, a veteran healthcare marketing strategist, they founded Always Culture. Together, they combined clinical expertise, operational insight, and broadcast-level storytelling to create a training solution that not only moves the needle on data but is genuinely engaging for the staff who watch it.

Leadership & Expertise

Always Culture was built on a foundation of diverse, high-level expertise. Our leadership combines decades of clinical practice, operational management in elite medical centers, and strategic healthcare consulting. This unique blend of perspectives ensures that our training is not only clinically accurate but operationally feasible and strategically sound.

Matt Dufilho | Co-Founder & Lead Producer

Matt bridges the often-separate worlds of clinical care and media communication. His background is deeply rooted in patient care, having spent over five years as a Physical Therapist in acute and post-surgical settings, including specialized work in burn and wound care at a Level 1 Trauma Center. Following his clinical career, he served for over a decade as the Director of Media Production & Design at UMC Health System. There, he spearheaded campaigns that communicated the system’s mission and built a culture of service. This dual experience allows Matt to create training content that respects clinical realities while utilizing broadcast-quality production to effectively engage staff.

Matt Brannon | Co-Founder

Matt Brannon brings a rigorous operational focus to patient experience. During his five-year tenure as a supervisor at Cedars-Sinai in Los Angeles, he oversaw daily operations for a high-volume, high-profile clinic. His leadership was defined by a commitment to supporting caregivers and support staff with the resources they needed to excel. Under his supervision, patient satisfaction data rose significantly, consistently exceeding national averages. Matt’s expertise ensures that Always Culture’s solutions are not just theoretical, but are designed to function within the complex, fast-paced operations of modern healthcare facilities.

Casey O’Quinn | Co-Founder

With over 25 years of experience as a marketing professional, business strategist, and consultant, Casey O’Quinn has dedicated his career to helping healthcare organizations grow with clarity and purpose. His work spans healthcare systems, hospitals, and patient-facing organizations, where he has focused on aligning marketing, education, and operational strategy with measurable outcomes, especially those tied to patient experience and quality of care.

Our Approach: Beyond the Hospital Walls

While Always Culture began with a laser focus on HCAHPS improvement for acute care hospitals, the need for clear, compassionate communication is universal across the healthcare continuum. We recognize that the patient journey rarely begins or ends in a hospital bed; it spans outpatient clinics, ambulatory surgery centers (ASCs), and post-acute care settings.

As the industry shifts toward Value-Based Purchasing (VBP) and increased transparency through CMS Star Ratings, the pressure to deliver exceptional experiences is no longer limited to inpatient leadership. That is why our training solutions are designed to be adaptable and scalable.

We are actively expanding our robust content library to support:

  • Outpatient & Ambulatory Surgery: Targeting specific OAS-CAHPS domains to assist independent centers in improving patient safety perception and communication scores.
  • Clinicians & Groups: Addressing CG-CAHPS metrics to ensure the patient experience is consistent from the initial consult to the procedure and recovery.

Whether delivered through HealthStream, our dedicated LMS, or integrated into your organization’s internal system, our approach remains the same: we provide high-quality, micro-learning modules that respect the staff’s time and intelligence. By focusing on universal principles of human interaction—responsiveness, clarity on medications, and discharge transitions—we help organizations of all types improve clinical outcomes by improving the communication that surrounds them.

The Bottom Line

Always Culture is the modern alternative to traditional, punitive compliance training. We exist to prove that when you treat your frontline staff with respect and equip them with evidence-based tools, both your culture and your data will improve.

We invite you to join the growing network of hospitals and healthcare organizations nationwide that have chosen to stop guessing at patient satisfaction and start training for it.
